Susan Jermstad: Registered Nurse, Certified Case Manager, CEO
Susan has over 25 years of experience working as a registered nurse and certified case manager. Her nursing experience includes case management in Home Health and Hospice. Susan held leadership roles as a Hospice Administrator and Director of Clinical Services for a Medical Management Company. Susan has experience in Managed Care as a Quality Practice Advisor, educating large provider practices and IPAs in appropriate HEDIS measures, medical record documentation guidelines and ICD-10 CPT coding in accordance with NCQA guidelines.
"Throughout my journey and career, I have always been drawn toward the senior population. My business, Chronic Care Plus, was created to work alongside physician groups who have the same passion and mission.
