RPM: CPT codes 99453, 99454, 99457, 99458: Remote Patient Monitoring services refer to the collection of patients generated health data, outside of a traditional clinical setting, which is stored and transmitted to a physician or other qualified healthcare professional for review and care management/treatment plan intervention. The requirements include the following elements:

• The patient must consent
• Devices must be registered with the FDA
• Order must be by a qualified medical professional
• Data must be wirelessly synchronized 24/7
